Enrolling for Pilates Classes

If you are interested in enrolling or booking a one to one session, I ask that you complete a health enrolment form before attending a session in order to ensure that you exercise safely. Please download the enrolment form, complete it and return it to me by email. If you can't download it, please contact me and I am happy to post a copy to you.
